scenario 2: Regional Support Teams are cut back
In this scenario, Sacramento County DHHS decides to continue funding the RSTs, but with significant budget cutbacks. If you've visited your local RST lately, you know that the money isn't going to plush offices, generous clerical support, and pleasant waiting rooms. Most are operating at well over 200% of design capacity. Like a dancing bear, it isn't how well they deliver care; the miracle is that they are able to deliver care at all. Paring the budget further would unmercifully prolong the death of a system that ceased fulfilling its mission months ago.
In this scenario, SacDoc can be...
- a technology backbone that can be integrated into an extensively revamped existing RST structure to reduce costs and make physicians' working conditions tolerable.
- used to create a facility to which medical care could be outsourced. The RSTs could continue receiving reimbursement for it from the County and provide social, oversight, and accounting services. They would buy the medical care through SacDoc (perhaps on a PPPM basis) but bill Sacramento County back using whatever voodoo incantations the County uses to determine reimbursement.
- the basis of a complete replacement for the RSTs if their owners determine that they can no longer operate at reduced budget levels.
